NRG Oncology’s randomized Phase 3 NRG-CC017 trial compares temporally modulated pulsed radiation therapy (TMPRT) with standard external-beam radiation in newly diagnosed, IDH-wildtype, MGMT-unmethylated glioblastoma. Both groups receive temozolomide. TMPRT divides each radiation fraction into small pulses separated by pauses rather than delivering the fraction continuously; the investigational element is dose timing, not a new drug or an increase in prescribed total radiation dose.

A central question is whether pulsed delivery can delay deterioration in memory and thinking. The study also evaluates survival, quality of life, and safety. The treatment course may use a 3- or 6-week radiation schedule, followed by adjuvant temozolomide according to protocol. Standard postoperative treatment already uses radiation and temozolomide, although MGMT-unmethylated tumors are less responsive to temozolomide than methylated tumors. This trial tests whether changing the delivery pattern can improve the balance between tumor treatment and cognitive function; a benefit has not yet been established.
