Lanreotide depot/autogel is a long-acting somatostatin analog that targets somatostatin receptors (SSTRs) expressed on many pheochromocytomas/paragangliomas (PPGLs). In results reported as a medRxiv preprint, the multicenter single-arm Phase II LAMPARA trial (NCT03946527) enrolling patients with advanced/metastatic PPGL and recent progression (n=18; 78% SDHx-mutant) evaluated lanreotide 120mg SC q4w, which was well tolerated (no discontinuations due to toxicity; mostly grade 1–2 AEs; diarrhea 67%, injection-site reaction 44%) and produced disease stabilization in 11 of 13 patients with an available assessment at approximately one year (85% of those assessed, not 85% of all 18 enrolled), with 2 partial responses emerging on extended follow-up; tumor growth-rate modeling suggested a long disease-control trajectory consistent with median PFS ≥2 years—an efficacy “shape” that looks more like a low-toxicity cytostatic option than PRRT (which more often yields objective responses but with radiation-associated hematologic/renal risk) and could be used to defer more intensive therapies in SSTR-positive disease.

Take home

If this signal holds in larger datasets, lanreotide could become a “low-friction” first step for SSTR-positive, slow-to-moderate tempo metastatic PPGL—especially appealing for SDHx-driven disease—buying time before PRRT, radiopharmaceuticals, TKIs, or cytotoxic chemo are needed.
