POLASTAR (jRCT1071220082) is a prospective, multicenter Japanese real-world study of polatuzumab vedotin (anti-CD79b antibody–drug conjugate delivering MMAE) plus rituximab, cyclophosphamide, doxorubicin, and prednisolone (Pola-R-CHP) in previously untreated DLBCL, intentionally including patients underrepresented in POLARIX such as those >80 years. In the full analysis set (n=485; median age 71 years; 73.7% IPI 2–5), investigator-assessed ORR was 93.2% and CR rate 83.3% (vs POLARIX end-of-treatment CR ~75% with Pola-R-CHP and ~72% with R-CHOP; PFS benefit HR 0.73), and in patients >80 years ORR/CR were 87.3%/80.0%. Grade ≥3 AEs occurred in 47.4% (56.7% if >80 years) with neutrophil count decreased 25.6% (26.7% if >80 years)—broadly in-family with POLARIX grade 3–4 neutropenia ~28% (Pola-R-CHP) and ~31% (R-CHOP); 1.0% had AE-related deaths and 6.8% discontinued polatuzumab due to AEs, with no new safety signals reported.

Take home

If durability tracks with POLARIX (PFS HR 0.73 vs R-CHOP), these real-world Japanese data—especially the workable safety/response profile in patients >80—support Pola-R-CHP as a pragmatic frontline backbone, but we still need time-to-event outcomes (PFS/OS) to judge whether the higher CR rate here reflects deeper benefit or just assessment/timing differences.
