Glofitamab is a CD3×CD20 T-cell–engaging bispecific antibody added to polatuzumab vedotin (CD79b ADC) plus R-CHP chemotherapy (rituximab, cyclophosphamide, doxorubicin, prednisone) in a multicenter Phase II frontline DLBCL study enrolling IPI≥2 patients (n=41; median age 61; 86% stage III/IV; 61% IPI 3–4; 12% MYC/BCL2/BCL6-rearranged HGBCL). Patients initially received two cycles of pola-R-CHP (one cycle of R-CHOP was permitted), with glofitamab added during cycles 3–6 and continued alone during cycles 7–8. Best CR was 95% (39/41; 95% CI 83–99) and at median follow-up 23.9 months median PFS was not reached with 24‑month PFS 95% (95% CI 89–100) (NCT05800366); by comparison, POLARIX (IPI 2–5) reported 2‑year PFS 76.7% with pola-R-CHP vs 70.2% with R-CHOP (HR 0.73), and historical R-CHOP outcomes in similar-risk populations are typically ~70–76% 2‑year PFS. Immune-effector toxicity was limited, with CRS in 10% of patients (all grade 1) and no reported ICANS, consistent with the delayed introduction of glofitamab. Cytopenias and other chemotherapy-related toxicities remain important considerations.

If this durability holds in larger/controlled datasets, adding a CD3×CD20 bispecific to pola-R-CHP could meaningfully raise the cure fraction in IPI≥2 DLBCL—though a single-arm (n=41) 95% 24‑month PFS result needs a randomized reality check against modern pola-R-CHP outcomes and risk-mix (IPI 2 vs 3–5) before calling it a new frontline standard.
