This Phase 2 BEDROCK study tests whether adding epcoritamab (Epkinly; a subcutaneous CD3×CD20 bispecific antibody that redirects T cells to kill CD20+ B‑cell lymphoma) can improve outcomes when layered onto dose‑adjusted R‑EPOCH (etoposide topoisomerase II inhibitor, doxorubicin anthracycline, vincristine microtubule inhibitor, cyclophosphamide DNA‑alkylator, prednisone glucocorticoid, plus rituximab anti‑CD20 mAb). The design includes an investigational epcoritamab+DA‑EPOCH‑R cohort and a DA‑EPOCH‑R comparison cohort, with 21‑day cycles for up to 6 cycles (patients who already received 1 cycle off‑study may receive 5 on‑study). The sponsor is the AIDS Malignancy Consortium; epcoritamab is co-developed by Genmab and AbbVie. The key angle is bringing a CD3×CD20 T‑cell–engaging modality into the frontline/early‑line setting for a very aggressive lymphoma where cure is the goal but relapse can be hard to salvage. Today’s care for high‑risk Burkitt lymphoma is intensive, short‑course multi‑agent chemoimmunotherapy regimens such as DA‑EPOCH‑R or CODOX‑M/IVAC with CNS evaluation/prophylaxis as indicated; this trial asks whether immunologic “add‑on” cytotoxicity can deepen remissions without derailing deliverability.
