MD Anderson Cancer Center is running a Phase 1, two-part (dose escalation then expansion) study testing LNK001, an autologous CAR T-cell therapy engineered as an AND-gate that requires dual recognition of ENPP3 and CAIX (CA9) to activate tumor killing—intended to improve tumor selectivity versus single-antigen CARs in kidney cancer. Patients first receive lymphodepleting chemotherapy with cyclophosphamide (DNA crosslinking alkylator) plus fludarabine (purine analog DNA synthesis inhibitor), followed by LNK001 infusion. The escalation portion defines dose-limiting toxicities and selects the recommended dose; the expansion then evaluates preliminary efficacy (including response rate and progression-free survival) in advanced/metastatic clear cell RCC with CAIX expression ≥50%, after at least one prior systemic line and typically after prior PD-1/PD-L1 immunotherapy plus a VEGFR TKI, including patients progressing on therapy or stopping for intolerance. The key angle is translating logic-gated cell therapy—common in hematologic cancers—into solid tumors where antigen overlap with normal tissues has historically limited CAIX-directed CAR programs. Today’s care for 2nd line+ metastatic ccRCC is sequential systemic therapy such as VEGFR TKIs (cabozantinib, axitinib, tivozanib), IO/TKI combinations earlier in disease course, and later options like belzutifan and lenvatinib/everolimus rather than cell therapy.
