Phase 2 nivolumab PD-1 checkpoint inhibitor plus AVD chemo in early-stage pediatric cHL

Trial ID
NCT07551583 on ClinicalTrials.gov
Cancer type
Hodgkin Lymphoma
Stage of research
Exploratory
Location
Houston, TX (single-site, UT MD Anderson)

This MD Anderson–sponsored Phase 2, open-label, single-arm study evaluates nivolumab (Bristol Myers Squibb; PD-1/CD279–blocking monoclonal antibody that reinvigorates anti-tumor T cells) combined with AVD chemotherapy: doxorubicin (anthracycline DNA intercalator/topoisomerase II inhibitor), vinblastine (microtubule inhibitor), and dacarbazine (alkylating/methylating agent). The primary goals are safety (CTCAE v5.0 adverse events) and preliminary efficacy using overall response rate, with secondary endpoints including duration of response, relapse-free survival, and overall survival. Eligible patients are ages 2–21 with newly diagnosed classical Hodgkin lymphoma, stage I–II, non-bulky disease (<10 cm), with organ-function and cardiac-function requirements. The key angle is bringing frontline PD-1 blockade into a young, highly curable population to potentially reduce the intensity and late effects associated with traditional approaches. Today’s care for early-stage pediatric/AYA cHL commonly relies on multi-agent chemo with response-adapted involved-site radiation for some patients, balancing cure rates against long-term toxicity risks.
