This Phase 1, multicenter, open-label study from Dispatch Biotherapeutics tests DISP-10, a combination of DV-10 (an adenovirus-based tumor-targeted viral therapy intended to inflame tumors and/or introduce antigenic signals) plus idecabtagene vicleucel (ide-cel; Abecma), an autologous BCMA-directed CAR T-cell therapy originally developed by Bristol Myers Squibb and 2seventy bio for multiple myeloma. The trial runs in two parts: Part 1 dose-escalation to determine a recommended dose for expansion, and Part 2 dose-expansion to further evaluate safety and preliminary efficacy at that dose. Adults with advanced/metastatic GI cancers are eligible, including colorectal adenocarcinoma and gastric/EGJ adenocarcinoma cohorts. The scientific angle is “solid-tumor CAR T enabling”: pairing a virus-based intratumoral/regionally delivered immune-priming component with a validated CAR T platform to address common barriers in solid tumors (trafficking, antigen density/heterogeneity, and immunosuppressive microenvironment). Today’s care for metastatic colorectal and gastric/EGJ adenocarcinoma is multi-agent systemic therapy (fluoropyrimidine + oxaliplatin/irinotecan, targeted therapy by biomarkers such as HER2/EGFR/BRAF, and checkpoint inhibitors for MSI-H/dMMR), with later lines often relying on additional chemotherapy and targeted agents rather than cellular therapy.
