This SWOG Cancer Research Network Phase 3 double-blind, placebo-controlled trial (BioFront; also listed as SWOG S2419) tests whether MO-03—a live biotherapeutic containing a butyrate‑producing bacterium Clostridium butyricum intended to modulate the gut microbiome—improves outcomes when added to investigator’s-choice standard immunotherapy-based first-line regimens for advanced/metastatic clear-cell renal cell carcinoma. Patients are randomized to MO-03 vs placebo alongside commonly used frontline combinations that include Bristol Myers Squibb’s nivolumab (PD‑1 antibody) plus ipilimumab (CTLA‑4 antibody) and/or VEGF-pathway TKIs such as Pfizer’s axitinib (VEGFR-1/2/3 inhibitor), Exelixis/Ipsen/Takeda’s cabozantinib (MET/AXL/VEGFR2 inhibitor), and Eisai/Merck’s lenvatinib (multikinase VEGFR/FGFR inhibitor) with Merck’s pembrolizumab (PD‑1 antibody). The scientific angle is whether a defined “microbiome add-on” can reproducibly boost checkpoint inhibitor benefit in a setting where response heterogeneity remains substantial. Today’s care for frontline advanced ccRCC is immunotherapy-based combinations (IO/IO or IO/TKI), with later-line switches among TKIs and other agents after progression.
