BeOne Medicines’ Phase 3 open-label, randomized study (BGB-11417-304; CELESTIAL‑TNCLL‑2) tests whether fixed-duration sonrotoclax + zanubrutinib can improve outcomes versus a fixed-duration control of venetoclax + acalabrutinib in adults with previously untreated chronic lymphocytic leukemia who require therapy per iwCLL criteria. Sonrotoclax and venetoclax are oral BCL2 inhibitors (pro-apoptotic therapy), while zanubrutinib and AstraZeneca’s acalabrutinib are oral BTK inhibitors (blocking B‑cell receptor signaling). The design focuses on depth and durability of remission with time-limited therapy—commonly assessed via undetectable measurable residual disease (uMRD) after treatment completion and progression-free survival—rather than indefinite treatment. Commercially, it’s a direct “best-in-class” style contest: BeOne is pairing its next-generation BCL2 inhibitor with its approved BTK inhibitor BRUKINSA to challenge an established BCL2+BTK approach. Today’s frontline CLL care is typically continuous BTK inhibitor therapy (e.g., acalabrutinib or zanubrutinib) or fixed-duration venetoclax-based regimens (often with an anti-CD20 antibody), and this trial asks whether a new fixed-duration BCL2 partner can raise uMRD rates and extend PFS with acceptable safety.
