Ibrutinib (BTK inhibitor) + venetoclax (BCL-2 inhibitor) improves long-term OS/PFS in 1L CLL, with more progression-free time without grade 3/4 AEs

Fixed-duration ibrutinib-venetoclax is an all-oral BTK/BCL-2 doublet evaluated in the Phase 3 GLOW trial (NCT03462719) in older/comorbid, previously untreated CLL, randomized vs chlorambucil-obinutuzumab (n=106 vs n=105) at 67-month median follow-up. At 66 months, OS was 79.0% with ibrutinib-venetoclax vs 60.8% with chlorambucil-obinutuzumab (HR 0.46), and PFS remained superior (66-month PFS 51.7% vs 18.1%; HR 0.27) with a 77.4% reduction in risk of needing second-line therapy; patients achieving uMRD at end of treatment had better PFS than MRD+ patients, particularly in unmutated IGHV. A tolerability-focused analysis also favored the doublet: restricted-mean time spent progression-free without grade 3/4 treatment-emergent AEs was 51.6 vs 30.2 months (difference +21.4 months). This integrates disease control with time spent experiencing severe toxicity; it is not median PFS, a quality-of-life score, or necessarily one uninterrupted toxicity-free interval. For contemporary context, time-limited first-line options for older/comorbid CLL include venetoclax-obinutuzumab (CLL14), rather than the chlorambucil-obinutuzumab control used here.

Take home

GLOW’s approximately 5.5-year follow-up supports durable disease control with fixed-duration ibrutinib-venetoclax and more total progression-free time without grade 3/4 treatment-emergent toxicity than chlorambucil-obinutuzumab. The practical question remains how it compares with contemporary time-limited venetoclax-obinutuzumab, and whether an ibrutinib-based regimen is appropriate for patients with cardiovascular risk.
