This randomized Phase 3 ECOG-ACRIN study (PTCL-STAT; EA4232) asks whether consolidative high-dose chemotherapy followed by autologous stem cell transplant (ASCT) improves outcomes versus observation alone for peripheral T-cell lymphoma (PTCL) patients who reach a first complete remission (CR1) after induction chemotherapy, including ALK-negative anaplastic large cell lymphoma. Patients are randomized to Arm I observation with imaging/biospecimens versus Arm II standard stem-cell mobilization/leukapheresis, then high-dose cytotoxic conditioning chemotherapy (mechanism: non-targeted DNA damage/cell-kill to eradicate residual lymphoma and ablate marrow) followed by reinfusion of the patient’s own hematopoietic stem cells to restore blood formation. The commercial/scientific angle is “practice-defining”: ASCT-in-CR1 is widely used in PTCL based on non-randomized and retrospective datasets, and this trial aims to settle whether the added toxicity/logistics of transplant truly buys longer progression-free or overall survival. Today’s care after CR1 varies by subtype and center, but commonly includes observation or consolidation with high-dose therapy/ASCT in fit patients after CHOP-like induction, with brentuximab vedotin (Seagen/Pfizer) incorporated up front for CD30-positive disease in many practices.
