Relacorilant is an oral, selective glucocorticoid receptor (GR) antagonist intended to reverse GR-driven chemotherapy resistance; in the open-label Phase 3 ROSELLA trial (GOG-3073/ENGOT-ov72; NCT05257408), relacorilant (150mg PO day −1/0/+1) plus weekly nab-paclitaxel (80mg/m² d1/8/15 q28d) was compared with nab-paclitaxel alone (100mg/m²) in platinum-resistant ovarian cancer after 1–3 prior lines (n=381; all had prior bevacizumab; 61% prior PARP inhibitor). At median follow-up 24.8 months, OS improved to 16.0 vs 11.9 months (HR 0.65, p=0.0004) with 18‑mo OS 46% vs 27%; this lands in the same OS neighborhood as mirvetuximab soravtansine’s Phase 3 MIRASOL in FRα‑high PROC (mOS 16.46 vs 12.75 months; HR 0.67) but without biomarker selection. Common AEs on the combo were neutropenia (64%), anemia (61%), fatigue (54%), nausea (44%), and no new safety signals were reported with longer follow-up; subsequent therapies were similar between arms.

Take home

If regulators buy the “no biomarker needed” story, ROSELLA’s ~4‑month OS lift could make relacorilant+nab‑paclitaxel a broadly applicable option in PROC—potentially competing with (or complementing) FRα‑selected mirvetuximab, but we’ll want to see how QoL and real-world tolerability shake out given the high hematologic AE burden typical of weekly taxanes.
