This randomized Phase 2 NCI/NRG Oncology study tests sacituzumab govitecan-hziy (Trodelvy, Gilead Sciences) plus bevacizumab against carboplatin and pegylated liposomal doxorubicin, or protocol-allowed gemcitabine, plus bevacizumab in recurrent platinum-sensitive ovarian, fallopian tube, or primary peritoneal cancer. Sacituzumab govitecan targets TROP2 and delivers SN-38, a topoisomerase I inhibitor; bevacizumab is an anti-VEGF antibody.

Participants must have BRCA1/2-mutated or HRD-positive disease that progressed during first-line PARP-inhibitor maintenance, with no intervening treatment, and progression at least 6 months after the last platinum dose. Both groups subsequently receive bevacizumab maintenance as specified by protocol. Progression-free survival is the primary endpoint, with response, survival, and safety also assessed. The comparison asks whether a TROP2-directed ADC can improve on a clinically relevant platinum combination in this defined post-PARP-inhibitor setting, where recurrent disease would otherwise commonly return to platinum-based therapy.
