Evorpacept (ALX148) is a high-affinity CD47 blocker with an inactive Fc to promote macrophage phagocytosis while limiting cytopenias; added to lenalidomide and rituximab (R2) in a single-arm Phase I (n=20; ≥2 prior lines; 90% indolent; NCT05025800), it produced 80% CR and 2-yr PFS 69%, with grade≥3 AEs of neutropenia 60%, infections 30%, and ALT increase 15% (no DLTs). Set against R2 alone—AUGMENT’s median PFS 39.4 mo with grade≥3 neutropenia 50% and MAGNIFY’s CR ~42%—the depth of remission looks higher while durability sits within the R2 range, acknowledging small, nonrandomized data.

Take home

If larger cohorts maintain deep CRs without prohibitive myelosuppression, CD47 blockade could make R2 a more potent chemo-free backbone in late-line indolent B-NHL.
