Epcoritamab is a subcutaneous CD3×CD20 bispecific antibody; here it was combined with lenalidomide (immunomodulatory agent) plus rituximab (anti‑CD20) as a chemotherapy-free regimen. In an adjusted comparative analysis, epcoritamab+R2 from EPCORE NHL-2 arm 2 (Phase 1b/2; NCT04663347; n=111; 23.4% US) was compared with “usual care” R2 or chemoimmunotherapy (CIT) drawn from COTA EHR data (n=380; US 100%) in patients with R/R FL after ≥1 prior line: ORR 96.9% vs 80.5% and CR 90.2% vs 55.1% (both P<.001), with improved time-to-event outcomes including PFS (HR 0.43, P<.001) and OS (HR 0.33, P=.003). Versus a key R2 benchmark from AUGMENT in R/R FL (ORR 78%, CR 34%, median PFS 39.4 months), the CR depth reported here (90.2%) looks substantially higher—but the caveat is this is not a randomized comparison and the control arm mixes R2 and CIT, so absolute cross-trial “delta” should be taken cautiously.

Take home

If these response-depth and hazard-ratio advantages hold up in cleaner (ideally randomized) comparisons against modern CIT/R2 sequencing, epcoritamab+R2 starts to look like a legitimate “chemo-sparing” way to buy long remissions earlier in the relapsed FL journey—though real-world adjustment can’t fully erase selection and measurement biases.
