Cabozantinib Alone Matches Cabo + Atezolizumab After 1L IO Combos in RCC with 10-month mPFS

Trial ID
NCT04338269 on ClinicalTrials.gov
Cancer type
Renal Cell Carcinoma

Cabozantinib, an oral VEGFR/MET/AXL TKI, monotherapy was compared with cabozantinib + atezolizumab (anti-PD-L1 antibody) in a CONTACT-03 (NCT04338269) subgroup of 236 advanced renal-cell carcinoma patients whose disease progressed on first-line IO-IO or IO-TKI regimens (cabo n=107; cabo+atezo n=129). Median PFS was nearly identical between cohorts—10.3 months with cabo vs 10.2 months with the doublet, and ORR was 36% and 37%. Toxicity, however, increased with the combination: grade 3/4 treatment-related AEs 58% vs 48%, serious AEs 25% vs 13%, and treatment discontinuation 17% vs 5%.

Take home

For patients relapsing after IO-based front-line therapy, cabozantinib monotherapy delivers solid 10-month PFS with fewer high-grade events—making the case that more PD-L1 blockade adds cost and toxicity without benefit in this setting.
