Lisocabtagene maraleucel (liso-cel) is an autologous anti-CD19 CAR-T cell therapy evaluated in the Phase 2 TRANSCEND FL study in relapsed/refractory follicular lymphoma after ≥2 prior systemic regimens including anti-CD20 and an alkylator (3L+; 107 treated, 103 efficacy-evaluable; NCT04245839). At median follow-up 41.5 months, IRC-assessed ORR was 97% (95% CI 92–99) with 94% CR (88–98); 36-month DOR 70%, PFS 68%, time free from next treatment 75%, and OS 86% (medians not reached), including similar durability in POD24, bulky disease, and double-refractory subgroups. Longitudinal safety showed declining grade ≥3 cytopenias and hypogammaglobulinemia over time, supportive care largely confined to the first 3 months post-infusion, and consistently low grade ≥3 infection rates—contrasting with non-curative 3L+ “chemo/targeted” options where real-world benchmark PFS is often ~1 year and with the other approved CD19 CAR-T axi-cel showing an estimated 36-month PFS ~54% in FL.

Take home

If this durability holds in broader real-world use, a one-and-done CD19 CAR-T course that keeps ~2/3 of heavily pretreated FL patients progression-free at 3 years could increasingly displace serial therapies—though the real test is whether centers can deliver it safely and equitably outside trial infrastructure.
