Ociperlimab (TIGIT) + tislelizumab (PD-1) posts ~55% ORR in 1L NSCLC cohorts; 66% ORR in 1L ES-SCLC

Ociperlimab is an anti-TIGIT antibody combined here with tislelizumab, an anti–PD-1 antibody engineered to reduce Fcγ receptor binding; in AdvanTIG-105 (Phase I/Ib, open-label; NCT04047862), patients received ociperlimab 900mg + tislelizumab 200mg Q3W ± 4 cycles of platinum-doublet chemotherapy, then maintenance. In untreated cohorts, ORR was 55.0% in sqNSCLC (40/41 evaluable), 54.8% in non-sqNSCLC (42/43), 40.0% in PD-L1+ NSCLC (45/45), and 65.9% in ES-SCLC (41/43), with mPFS 8.2, 11.3, 5.5, and 4.9 months respectively; in checkpoint inhibitor–experienced NSCLC, ORR was 7.7% with mPFS 2.9 months. Grade 3–4 TR-TEAEs occurred in 58.5% (sqNSCLC), 60.5% (non-sqNSCLC), 20.0% (PD-L1+), 23.1% (IO-experienced), and 53.5% (ES-SCLC)—benchmarks to keep in mind include pembrolizumab + carbo/(nab-)paclitaxel in 1L metastatic sqNSCLC (KEYNOTE-407) with ORR 57.9% and mPFS 6.4 months, pembrolizumab + pemetrexed/platinum in 1L metastatic non-sqNSCLC (KEYNOTE-189) with ORR ~48% and mPFS 8.8 months, and atezolizumab + carboplatin/etoposide in 1L ES-SCLC (IMpower133) with ORR 60% and mPFS 5.2 months.

Take home

Ociperlimab + tislelizumab ± chemotherapy showed clear activity in untreated NSCLC, but the response rates and PFS are broadly in the range of existing chemo-immunotherapy rather than clearly better. The 65.9% ORR in ES-SCLC is notable, but median PFS of 4.9 months and DOR of 4.1 months do not yet suggest improved durability. Randomized trials will be needed to show whether adding TIGIT blockade materially improves outcomes over current PD-(L)1-based treatment.
