Ociperlimab (anti‑TIGIT) + tislelizumab (anti‑PD‑1) enriches responses in PD‑L1/TIGIT “double‑high” 1L metastatic NSCLC

Trial ID
NCT04047862 on ClinicalTrials.gov
Cancer type
Non-Small Cell Lung Cancer (NSCLC)

Ociperlimab (OCI) is an anti‑TIGIT monoclonal antibody combined with tislelizumab (TIS), an anti‑PD‑1 antibody, in AdvanTIG‑105 (Phase 1/1b, open‑label) in treatment‑naïve metastatic PD‑L1+ (TC ≥1%) NSCLC (NCT04047862). In biomarker‑evaluable patients (n=113; Aug 2024 cutoff), overall mPFS was 5.5mo with ORR 34.5%, but efficacy enriched with higher baseline PD‑L1 and TIGIT: PD‑L1 ≥25% vs <25% mPFS 6.9 vs 4.2mo (HR 0.58, 95% CI 0.37–0.90) with ORR 47% vs 22%; TIGIT ≥5% vs <5% mPFS 6.8 vs 4.1mo (HR 0.64, 95% CI 0.41–1.00) with ORR 45% vs 19%; PD‑L1/TIGIT double‑high vs other mPFS 8.3 vs 4.2mo (HR 0.53, 95% CI 0.33–0.86) with ORR 55% vs 22%. The “signal” looked more histology‑dependent in non‑squamous NSCLC (e.g., PD‑L1 ≥25% mPFS 8.3 vs 4.2mo; HR 0.55) than squamous NSCLC (PD‑L1 ≥25% mPFS 5.5 vs 5.3mo; HR 0.63), and mechanistically aligned GEP signatures (NK/Treg/macrophage) trended with longer PFS mainly in non‑squamous; safety was not reported in this biomarker abstract, and the absolute mPFS for the overall biomarker‑evaluable set (5.5mo) sits below what PD‑1 monotherapy can deliver in the highest PD‑L1 populations (e.g., cemiplimab PD‑L1 ≥50% mPFS 8.1mo; ORR 46.5%).
