Linvoseltamab is an off-the-shelf T-cell–engaging BCMA×CD3 bispecific antibody evaluated in LINKER-MM1 (Phase I/II; NCT03761108) in triple-class–exposed relapsed/refractory multiple myeloma (n=117; median follow-up 21.3 months). Patient-reported outcomes (EORTC QLQ-C30, QLQ-MY20, EQ-5D-3L) improved nominally vs baseline across multiple domains; Global Health Status/QoL, fatigue, and pain improved by ~week 16, and pain reached a clinically meaningful threshold by week 20 and was generally maintained through week 104, with benefits concentrated in responders (83 responders improved across scales; 34 nonresponders numerically worsened on most scales). While this HRQoL abstract is efficacy-adjacent rather than tumor-response–centric, it aligns with the longer-follow-up activity at the 200mg dose (ORR 71% with ≥CR 52%; median DoR 29.4 months; median PFS not reached), and the expected bispecific safety profile (CRS 46% with ~1% grade 3; ICANS 7.7% with ~2.6% grade 3; fatal infections reported) — broadly in the same “late-line BCMA bispecific” performance band as teclistamab (ORR ~63%) and elranatamab (ORR ~61%) but with cross-trial comparison caveats. Take Home: If a BCMA bispecific can deliver ~70% responses and keep pain/fatigue trending better (not worse) for responders out past a year, that’s a patient-relevant differentiator in late-line myeloma—now the bar is cleaner comparative HRQoL plus infection mitigation strategies that don’t blunt durability.
