Epcoritamab (CD3×CD20 bispecific) hits early MRD-negativity in R/R FL—PFS signal strengthens by C3D1

Epcoritamab is a subcutaneous CD3×CD20 T-cell–engaging bispecific antibody evaluated as monotherapy in the EPCORE® NHL-1 Phase 1/2 study (NCT03625037) in relapsed/refractory follicular lymphoma after ≥2 prior lines; this analysis used the clonoSEQ® next-generation sequencing assay to call MRD from PBMCs and/or plasma ctDNA. Most MRD-evaluable patients converted to MRD-negative by cycle 3 day 1, and MRD-negativity at the C3D1 landmark (by either PBMCs or ctDNA) correlated with prolonged PFS (median not reached) even when radiographic response status was ignored; among PET/CT responders at C3D1 and C5D1, MRD-positive patients had shorter PFS than MRD-negative patients. In multivariable landmark models (week 12 and week 18 PET/CT), MRD-negative responders (PBMC at week 12; both PBMC+ctDNA at week 18) retained significantly improved PFS after adjustment for baseline risk factors; safety/AE data were not reported in this MRD-focused abstract.

Take home

If MRD by ctDNA/PBMC can reliably flag “PET response but biologically not deep enough,” epcoritamab (and other CD3×CD20 bispecifics) may get a practical early on-treatment decision tool in ≥3L FL—though we still need the actual MRD-negative rate, hazard ratios, and follow-up duration to judge how much this beats today’s usual post–anti-CD20/lenalidomide sequencing.
