Glofitamab is an IV CD20×CD3 T-cell–engaging bispecific antibody; here it was combined with obinutuzumab/rituximab plus CHOP (G/R-CHOP) in a Phase 1b dose-escalation study in relapsed/refractory B-cell non-Hodgkin lymphoma previously exposed to ≥1 anti-CD20–containing regimen (NCT03467373). Thirty-one patients with follicular lymphoma (including transformed FL), mantle-cell lymphoma, or marginal-zone lymphoma received glofitamab beginning on C2D8, using fixed microdose or step-up dosing schedules up to 30 mg; the complete metabolic response rate was 83.9%, and median PFS was not reached after 44.2 months of follow-up. Safety was substantial: grade 3/4 AEs occurred in 80.6% and grade 5 AEs in 12.9%, mostly from infections; CRS occurred in 51.6%, including grade 3/4 CRS in 12.9%. Because the cohort spans several biologically and clinically distinct lymphoma subtypes with different modern treatment pathways, there is no clean single SOC benchmark against which to compare the response rate or durability.

Take home

This is interesting proof-of-concept that CD20×CD3 T-cell engagement can be layered onto immunochemotherapy and produce deep, potentially durable remissions across several R/R B-cell lymphomas—but the heterogeneous 31-patient cohort makes efficacy comparisons difficult, while the infection-related deaths and double-digit high-grade CRS make safety a central issue for any larger development program.
