Niraparib, an oral PARP inhibitor, paired with the anti-VEGF antibody bevacizumab was evaluated in the single-arm phase 2 OVARIO trial (NCT03326193). Among 105 stage IIIB-IV patients who achieved at least a partial response to front-line platinum + bevacizumab, the 18-month progression-free survival rate hit 62% and median PFS reached 19.6 months. After 65.7 months’ follow-up, median overall survival was 61.1 months with 52% data maturity; OS was not reached in the HRD subgroup but fell to 38.7 months in HR-proficient disease. Grade 3-4 events were mostly hematologic, and no new safety signals emerged. For context, randomized PAOLA-1 reported a median OS of 56.5 months for olaparib + bevacizumab and 51.6 months for bevacizumab alone, while bevacizumab maintenance by itself showed ~43 months in high-risk patients in GOG-0218.

Take home

Although cross-trial caveats apply, these long-term data hint that adding niraparib to bevacizumab could edge past other maintenance strategies—especially in HRD-positive tumors—but randomized confirmation is needed before practice changes.
