Ibrutinib (BTK inhibitor) + venetoclax (BCL-2 inhibitor) delivers 95% ORR and 40.2-mo mPFS in 1L non-blastoid MCL

Cancer type
Mantle Cell Lymphoma (MCL)

Ibrutinib is an oral Bruton’s tyrosine kinase (BTK) inhibitor and venetoclax is an oral BCL-2 inhibitor; in the Phase 3 SYMPATICO open-label first-line cohort (NCT03112174), patients with previously untreated non-blastoid mantle cell lymphoma who were ≥65 years (n=65) or had TP53 mutation (n=11; total enrolled n=78) received ibrutinib 560 mg QD + venetoclax ramp-up to 400 mg QD for 2 years, then ibrutinib alone until progression/toxicity. At median 40.5 months on study, CR was 69% and ORR 95%, with median DoR 37.1 months, mPFS 40.2 months, and OS not reached (3-year OS 79%); the ≥65/TP53m subgroup underperformed (CR 44%, mPFS 22.0 mo, 3-year OS 66%) vs ≥65/no TP53m (CR 76%, mPFS 40.2 mo, 3-year OS 85%), consistent with TP53m being a high-risk biology (TP53 mutations reported ~27% at baseline in meta-analytic datasets). Toxicity was dominated by diarrhea (49%), fatigue (37%), neutropenia (35%), and COVID-19 (32%); efficacy benchmarks look competitive with historical BR (median PFS ~35 months) but below chemoimmunotherapy + BTK intensification in SHINE (ibrutinib+BR median PFS 80.6 months), with the caveat that cross-trial comparisons are messy and this cohort enriches for higher-risk patients (older/TP53m).

Take home

If a chemo-free ibrutinib/venetoclax backbone can consistently hold ~3+ years of median PFS in older, non-blastoid MCL—while not “fixing” TP53m—it could still be a pragmatic front-line option for patients who want to avoid bendamustine, but it will need clearer comparative data (and modern infection-risk mitigation) to justify displacing BR- or BR+BTK-based standards.
