Ibrutinib is an oral Bruton tyrosine kinase (BTK) inhibitor combined here with anti-CD20 rituximab as a chemotherapy-free first-line regimen. In the randomized, double-blind Phase 3 PERSPECTIVE trial (NCT02947347) in previously untreated follicular lymphoma requiring therapy by GELF criteria and deemed ineligible for chemoimmunotherapy due to age/comorbidities (n=445; 3:1 randomization), ibrutinib+rituximab improved investigator-assessed PFS vs placebo+rituximab (median 42.0 vs 32.8 months; HR 0.713, p=0.023) and increased ORR (81% vs 68%) with a small CR lift (31% vs 26%), but OS was immature and not different (HR 1.121, p=0.55). Safety cost was meaningful: grade ≥3 AEs were 78% vs 57% (notably neutropenia 16% vs 7%, pneumonia 9% vs 5%, hypertension 8% vs 5%, diarrhea 6% vs 2%, and higher COVID-19/COVID pneumonia), which is a tougher profile than rituximab-based low-intensity approaches and doesn’t obviously beat modern frontline chemoimmunotherapy/R2 benchmarks where median PFS commonly lands ~6–10 years in fitter trial populations.

Take home

If you truly have a frail, chemo-ineligible FL population, this is a real (but modest) ~9-month median PFS gain over rituximab alone—yet with BTK-typical infectious/cardiovascular toxicity and no OS signal so far, the bar for “practice-changing” likely hinges on better-tolerated next-gen BTK strategies or a clearer quality-of-life/time-to-next-treatment advantage.
