Entrectinib, an oral TRK/ROS1 inhibitor, was pooled across the phase 1–2 STARTRK-NG (NCT02650401), TAPISTRY (NCT04589845), and STARTRK-2 (NCT02568267) trials. Across 26 ROS1-fusion patients (<18 y) there was a 69% objective response rate (15% CR; 54% PR) with median response time of 1.8 mo, and median OS not reached. Grade 3/4 treatment-related events were mainly weight gain, anemia, and fractures (23%).
Take home
These rapid, durable responses underscore the value of routine ROS1 testing in pediatric solid and CNS tumors, mostly glioma at 0.3% to 1% of cancers—and spotlight bone-health monitoring as entrectinib usage expands.
