Lisocabtagene maraleucel (liso-cel) is an autologous CD19-directed CAR-T cell therapy evaluated in the Phase 1 TRANSCEND NHL 001 program in relapsed/refractory large B-cell lymphoma (R/R LBCL) after ≥2 prior lines. Across the TRANSCEND efficacy-evaluable set (n=257; 345 leukapheresed, 270 infused), median OS was 27.5 months with an estimated 5-year OS rate of 38% (leukapheresed set: mOS 15.2 months; 5-year OS 33%); median disease-specific survival (DSS) was 67.8 months with 5-year DSS 52%. In the subset alive at end-of-study who entered long-term follow-up (LTFU; n=84), median OS/DSS were not reached with estimated 5-year OS 78% and 5-year DSS 92%; most deaths occurred within 2 years post-infusion, and investigators report no new late safety signals with low rates of late severe infections and second primary malignancies (NCT02631044; LTFU NCT03435796)—a markedly better long-term survival profile than historical chemo-based salvage benchmarks in refractory DLBCL (e.g., SCHOLAR-1 ORR 26%/CR 7% and mOS ~6.3 months).

Take home

If the “most deaths by ≤2 years” pattern holds across real-world practice, liso-cel’s 5-year OS ~38% looks like a genuine long-term remission/plateau signal in a setting where conventional post–≥2L salvage historically delivered low CR rates and single-digit-month median OS—though the especially strong LTFU subset outcomes are inherently conditional on surviving long enough to enroll.
