CA125-Targeted Oregovomab Added to Carboplatin/Paclitaxel Hits 10-Year OS 121 mo vs 65 mo in Optimally Debulked Stage III/IV Ovarian Cancer

Ovarian Cancer

Oregovomab is a monoclonal antibody that engages host immunity against the protein CA125, which is expressed in ovarian cancer cells. In the randomized QPT-ORE-002 Phase II trial (NCT01616303), 97 women with optimally resected stage III/IV epithelial ovarian cancer received front-line carboplatin-paclitaxel with oregovomab on cycles 1, 3 & 5 (CPO, n=47) or chemotherapy alone (CP, n=50). After a median 9.1-year follow-up, patients in the CPO group had a median progression-free survival (PFS) of 41.8 months, compared to 12.2 months in the CP group. (HR 0.46, p = 0.003) and median overall survival to 121.3 mo versus 64.7 mo (HR 0.47, p = 0.012) without added grade ≥3 toxicity.

Take home

If a Phase III study confirms these dramatic PFS and decade-level OS gains, oregovomab timed with chemo could become a new immune-chemo backbone for CA125-positive ovarian cancer.
