In a matching-adjusted indirect comparison of the EPCORE-NHL-1 FL cohort (≈127 pts) against real-world standard of care/chemoimmunotherapy (SOC/CIT) from SCHOLAR-5 and trial arms of two CD20xCD3 bispecific antibodies (mosunetuzumab in the GO29781 study and odronextamab in the ELM-2 study), subcutaneous epcoritamab (another CD20xCD3 bispecific antibody) producedORR 91% and CR 74%, far exceeding SOC/CIT (ORR 57%, CR 32%). Relative to peer CD20×CD3 bispecifics, ORR was higher vs mosunetuzumab (91% vs 80%) and odronextamab (92% vs 81%), with comparable CR rates. Notably, epcoritamab recorded no grade ≥3 CRS or any-grade ICANS, whereas grade ≥3 CRS occurred in 2–4% and ICANS in up to 4% with the IV comparators.

Take home

A cleaner safety profile and double-digit response advantage over both SOC chemo and peer bispecifics position epcoritamab as the subcutaneous front-runner for follicular lymphoma patients who’ve relapsed after two lines of therapy—pending direct head-to-head or CAR-T comparisons to confirm durability.
