In the AARON phase I/II trial, frontline non-fit AML patients with adverse risk profiles treated with azacitidine/venetoclax plus dual checkpoint blockade using nivolumab (anti-PD-1) and relatlimab (anti-LAG-3) achieved an impressive 70% CR/CRi rate among evaluable patients, while the regimen was tolerable with manageable immune adverse events; however, no responses were seen in the relapsed/refractory cohort.
These interim findings underscore that combining hypomethylating agents with PD-1/LAG-3 inhibition can offer a promising and safe treatment option for high-risk, non-fit AML patients, paving the way for further immunomonitoring to optimize therapeutic strategies.
