This MD Anderson Phase 1/2 study adds olaparib, a PARP1/2 inhibitor developed by AstraZeneca and Merck, to a sequential high-dose chemotherapy program for relapsed or refractory germ cell tumors. The first treatment sequence includes gemcitabine, docetaxel, and carboplatin (GemDC); the subsequent sequence uses carboplatin and etoposide (CE), with autologous stem-cell support as specified by the protocol. Both sequences therefore include platinum chemotherapy alongside PARP inhibition, with intensive inpatient treatment and stem-cell rescue.

The early-phase aim is to establish a tolerable olaparib dose in this intensive setting and assess preliminary disease-control outcomes, including the 2-year relapse-free survival rate. PARP inhibition is intended to increase vulnerability to chemotherapy-induced DNA damage, rather than serving as routine maintenance. Salvage treatment for germ cell tumors can still be curative in selected patients, and high-dose chemotherapy with stem-cell rescue is delivered at experienced centers. The trial tests whether olaparib can improve that platform without making marrow, organ, or other treatment-related toxicity unacceptable; it does not establish superiority over existing salvage approaches.
