AstraZeneca’s SERAFA-1 is a single-arm, open-label, multicenter Phase 3b study testing camizestrant (an oral selective estrogen receptor degrader that binds ER and promotes ER degradation, including in ESR1-mutant and wild-type disease) combined with Novartis’ ribociclib (a CDK4/6 inhibitor that arrests the cell cycle at the G1–S checkpoint). The aim is to characterize real-world-like effectiveness plus safety/tolerability when this combo is used as first-line systemic therapy for unresectable locally advanced or metastatic ER-positive, HER2-negative breast cancer, including patients with recurrent disease after prior endocrine therapy in the early setting. Participants remain on treatment until discontinuation for progression, toxicity, or other reasons, consistent with how CDK4/6-based regimens are used clinically. The key scientific/commercial angle is whether an oral next-generation SERD can credibly replace/upgrade the endocrine “backbone” paired with CDK4/6 inhibition, a space historically dominated by aromatase inhibitors or fulvestrant; today’s care for 1L ER+/HER2− advanced disease is endocrine therapy (typically an aromatase inhibitor or fulvestrant) plus a CDK4/6 inhibitor (ribociclib, palbociclib, or abemaciclib), with later lines guided by biomarkers such as PIK3CA, ESR1, and BRCA/PALB2.
