This Phase 2, open-label, single-arm study at the University of Miami evaluates pembrolizumab (Merck/MSD; PD-1–blocking monoclonal antibody that restores T-cell anti-tumor activity) combined with lenvatinib (Eisai; oral multi-kinase inhibitor that targets angiogenesis and growth signaling, including VEGFR and FGFR-family kinases) in recurrent endometrial cancer that is mismatch repair–proficient (the majority of endometrial cancers) after progression on first-line platinum doublet chemotherapy plus immunotherapy. Treatment continues up to ~24 months or until progression/toxicity, with total participation ~26 months. The scientific angle is testing whether this established anti–PD-1 + anti-angiogenic/TKI backbone retains benefit after prior first-line chemo-immunotherapy, a setting where post-progression options commonly revert to sequential single-agent chemotherapy (e.g., doxorubicin or paclitaxel), hormonal therapy in selected ER/PR+ tumors, and biomarker-directed options when present. The combo is already supported in previously treated advanced/recurrent disease versus physician’s-choice chemo in large randomized data, so this trial narrows in on the increasingly common “IO-exposed, pMMR” relapse population.
