Hoffmann‑La Roche/Genentech’s CEVOLUTION (Phase 3) is a randomized, open-label, multicenter study testing cevostamab + pomalidomide + dexamethasone (CevosPd) versus investigator’s choice standard regimens in active/measurable multiple myeloma after 1–3 prior lines, specifically requiring prior exposure to an anti‑CD38 monoclonal antibody and lenalidomide. Cevostamab is a T‑cell–redirecting bispecific antibody that binds FCRH5 on myeloma cells and CD3 on T cells to drive immune synapse–mediated cytotoxicity; pomalidomide is an IMiD that enhances anti-myeloma immunity and has direct tumor effects via cereblon-driven substrate degradation; dexamethasone is a glucocorticoid used for anti-myeloma activity and as premedication. The control arm uses familiar “post‑lenalidomide/post‑daratumumab” salvage options (e.g., pomalidomide/dexamethasone backbones with agents such as daratumumab, elotuzumab [SLAMF7], or carfilzomib [proteasome inhibitor], depending on local practice). The key commercial/scientific angle is whether an off‑the‑shelf bispecific against the highly myeloma-enriched antigen FCRH5 can move earlier (2nd–4th line) and displace today’s common sequencing of IMiD- and proteasome inhibitor–based triplets after anti‑CD38 and lenalidomide exposure.
