In this randomized, double-blind Phase 3 DAWN-303 trial from Incyte, patients with previously untreated metastatic pancreatic ductal adenocarcinoma (PDAC) whose tumors harbor a KRAS G12D mutation are assigned to INCB161734 or placebo, each given with investigator’s choice of first-line chemotherapy. INCB161734 is an oral, selective small-molecule inhibitor of KRAS G12D, aiming to directly shut down a dominant oncogenic driver in PDAC (KRAS mutations overall are present in the vast majority of PDAC, with G12D a common subtype). Participants receive either modified FOLFIRINOX (multi-agent cytotoxic chemotherapy) or gemcitabine + nab-paclitaxel (DNA synthesis inhibition plus microtubule inhibition), reflecting real-world first-line regimens. The key commercial/scientific angle is a registrational attempt to move a KRAS G12D inhibitor into frontline therapy rather than later-line salvage, potentially creating a biomarker-defined standard in a disease where outcomes remain poor. Today’s care for metastatic PDAC is first-line mFOLFIRINOX or gemcitabine/nab-paclitaxel followed by subsequent-line chemotherapy (and supportive care), with targeted options limited to smaller molecular subsets (e.g., BRCA/PALB2, MSI-H) rather than KRAS.
