Janssen Research & Development (Johnson & Johnson) is running this Phase 3 randomized head-to-head study (TRIlogy-5) to compare JNJ-79635322 versus teclistamab in relapsed/refractory multiple myeloma after 1–3 prior lines including an anti-CD38 antibody and lenalidomide. Arm A gives subcutaneous JNJ-79635322, a T-cell–redirecting trispecific monoclonal antibody that binds BCMA and GPRC5D on myeloma cells while engaging CD3 on T cells, with treatment intended for a finite duration or until intolerable toxicity. Arm B gives subcutaneous teclistamab (Johnson & Johnson), a BCMA×CD3 bispecific T-cell engager, continued until progression or intolerable toxicity. The commercial/scientific angle is a registrational, direct comparator against a widely used BCMA bispecific, testing whether dual-antigen targeting (BCMA + GPRC5D) can improve disease control and/or durability while remaining feasible in earlier relapse (2nd–4th line). In this setting, today’s care is sequential relapse therapy using triplets (often proteasome inhibitor- or pomalidomide-based), anti-CD38 re-treatment in selected patients, and increasingly T-cell–redirecting therapies (BCMA bispecifics or BCMA CAR-T) depending on eligibility, access, and prior exposure.
