Mayo Clinic evaluates heated intraperitoneal cisplatin/paclitaxel delivered intra-op alongside D2 gastrectomy to cut peritoneal relapse. Even with peri-op FLOT, peritoneal spread is the Achilles’ heel; p-HIPEC aims to eradicate microscopic disease at the source. DFS and peritoneal-recurrence rates, balanced against renal/neurotoxicity risk, will determine whether this strategy merits broader adoption.
