This pilot trial, sponsored by University of Hawaii, evaluates response-adapted SBRT and subsequent surgery following preoperative ipilimumab and nivolumab in patients with borderline resectable hepatocellular carcinoma, including higher-risk Native Hawaiian and Pacific Islander individuals. The study design features imaging-driven resection decisions and additional SBRT for unresectable cases, with biomarker analyses to guide personalized treatment strategies. Previously Ipi/Nivo produced 27% to 32% ORR in CheckMate 040, so hopefully this can convert more patients to a resectable state.
