This single-institution, single-arm feasibility trial evaluates the combination of systemic atezolizumab (a PD-L1 targeting monoclonal antibody) and bevacizumab (a VEGF targeting monoclonal antibody) with locoregional Y90 radioembolization for patients with newly diagnosed, high-risk hepatocellular carcinoma. Two cohorts include participants within Milan Criteria with AFP ≥400 ng/mL as bridge therapy to liver transplant and those beyond Milan Criteria as downstaging candidates under the oversight of Mount Sinai.
