This Phase 3 open-label, multicenter randomized trial compares tarlatamab plus durvalumab, carboplatin, and etoposide versus standard durvalumab, carboplatin, and etoposide in untreated stage IV extensive-stage small-cell lung cancer patients. Tarlatamab targets DLL3/CD3 while durvalumab inhibits PD-L1, and the trial evaluates whether adding tarlatamab can improve overall survival. Current SoC adds PD-L1 to platinum/etoposide (CASPIAN, IMpower133) with modest survival gains; DLL3-directed T-cell engagement could push OS further.
