This National Cancer Institute (NCI) MyeloMATCH Phase 2, randomized treatment substudy tests whether adding AbbVie/Genentech’s venetoclax (a small-molecule BCL-2 inhibitor that promotes apoptosis) to standard “7+3” induction can match or outperform adding Pfizer’s Mylotarg (gemtuzumab ozogamicin, a CD33-directed monoclonal antibody–calicheamicin antibody-drug conjugate that delivers a DNA-damaging payload to CD33+ blasts). Patients receive either 7+3 plus gemtuzumab ozogamicin (days 1 and 4) or 7+3 plus venetoclax (days 1–11, with shifted 7+3 start dates), then proceed to post-remission therapy per local standard. Eligible patients are younger adults with newly diagnosed core-binding factor AML (t(8;21), inv(16), or t(16;16)), a biologically defined subset where relapse prevention and depth of remission matter. The key scientific/commercial angle is whether venetoclax—already established in older/unfit AML regimens—can deliver a strong, chemotherapy-backbone benefit in a favorable-risk genomic AML subgroup, potentially competing with or complementing CD33-targeted intensification. Today’s care for fit, newly diagnosed CBF-AML is intensive induction with cytarabine + an anthracycline, often with gemtuzumab ozogamicin in CD33+ disease, followed by consolidation (commonly high-dose cytarabine-based approaches) and MRD-guided decisions.
