Petosemtamab + Pembrolizumab: Dual EGFR & LGR5 Targeting Yields 60% ORR in PD-L1+ r/mHNSCC

Trial ID
NCT06525220 on ClinicalTrials.gov
Cancer type
Head and Neck Cancer
Published in
New in Oncology - May 29, 2025

In this Phase 2 trial, treatment of 43 evaluable r/m PD-L1+ HNSCC patients with petosemtamab 1500 mg Q2W plus pembrolizumab produced a 60% overall response rate (5 complete responses; 11%) and a median duration of response of 11 months. The 6‐month overall survival rate was 93%, with manageable, predominantly Grade 1/2 adverse events and infusion-related reactions primarily at first infusion. Compared to pembrolizumab monotherapy (KEYNOTE-048 trial) ORR of 17% and CR of 5% makes this combo look even better!

Take home

It looks like the majority (80+%) of HNSCC is going to benefit from combinations like this one. We look forward to seeing the mature results in the Phase 3 trial, LiGeR-HN1 (NCT06525220). We’ll be sending our patients their way.
