GemCis + Lenvatinib + Adebrelimab (VEGFR/FGFR/PD-L1) Hits 46% ORR in iCCA

Trial ID
NCT06298968 on ClinicalTrials.gov
Cancer type
Cholangiocarcinoma (Bile Duct Cancer)
Published in
New in Oncology - July 16, 2026

Adebrelimab is a PD-L1 antibody and lenvatinib is an oral VEGFR/FGFR/RET/KIT multi-kinase inhibitor added to gemcitabine/cisplatin in a prospective, multicenter, single-arm Phase 2 first-line trial for unresectable/advanced intrahepatic cholangiocarcinoma (n=26; NCT06298968; conducted outside the US, with adebrelimab not US-approved). At median follow-up of 8.1 months, ORR was 46.2% (12/26), DCR was 92.3%, median PFS was 8.5 months, and median OS was not reached with only 4 deaths. Grade 3/4 TRAEs were mainly neutropenia (23.1%) and thrombocytopenia (19.2%) with no treatment-related deaths; the 8.5-month PFS is numerically above TOPAZ-1 durvalumab+GemCis PFS around 7.2 months, but OS is too immature to compare against PD-1/PD-L1+GemCis standards.

Take home

The triplet is active enough to watch, but adding lenvatinib to chemo-IO needs a cleaner safety/OS story before it can challenge simpler durvalumab- or pembrolizumab-based GemCis.
