Adebrelimab is an anti–PD-L1 IgG4 checkpoint inhibitor combined here with platinum–taxane chemotherapy (cisplatin + docetaxel) as 2-cycle neoadjuvant immunochemotherapy in a prospective, single-arm Phase II exploratory study in clinical stage IVB oral squamous cell carcinoma (NCT06277791). Among evaluable patients (n=25), RECIST activity was modest (ORR 32% with 0 CR; 10.7% PD), but surgical/pathologic endpoints were strong: MPR 60% and pCR 4% (pathological response rate 64%), with an R0 resection rate of 100%; at median 12.0 months’ follow-up, estimated 1-year OS was 87.9% (3 deaths). Safety looked manageable (grade 3 TRAEs 7.2%, no grade 4/5), and the 64% MPR/pCR rate appears well above historical induction chemotherapy pathologic response rates in OSCC (~28% reported with TPF induction in a randomized trial), though cross-trial comparisons are inherently noisy and IVB biology/surgical intent may differ.

Take home

If the pathologic signal holds up (and translates into durable locoregional control), this regimen could become a credible “make-it-resectable” neoadjuvant strategy for IVB oral cavity disease—but the low radiographic ORR and short follow-up mean we still need longer EFS/OS readouts and a comparator to know whether MPR here is truly moving the survival needle.
