Atorvastatin (HMG-CoA) + chemotherapy shows 70.3% tumor-marker declines in advanced PDAC

Trial ID
NCT06241352 on ClinicalTrials.gov
Cancer type
Pancreatic Ductal Adenocarcinoma (PDAC)

Atorvastatin, an HMG-CoA reductase inhibitor (statin), was paired with chemotherapy after a 260-line PDAC organoid biobank tied chemoresistance to cholesterol/glycosylation programs and showed statins could re-sensitize organoids. In a single-center, single-arm Phase 2 (NCT06241352) for advanced PDAC (n=37), 70.3% had >20% tumor-marker declines (and ~25% had >50% decrease); RECIST responses and detailed safety were not reported. For context, first-line FOLFIRINOX and nab-paclitaxel+gemcitabine yield RECIST ORR ≈31.6% and 23% with mOS 11.1 and 8.5 months, respectively—so these biomarker responses aren’t directly comparable but may indicate biological activity given CA19-9 decline’s association with survival.

Take home

If a randomized study confirms RECIST responses and survival benefit, a low-cost statin add-on—guided by organoid/multi-omics signatures—could carve out a niche for chemo-resistant PDAC. However, the effect, if any, is unlikely to be transformational despite an interesting individual response. From the article, “Based on the follow-up results obtained so far, the 26 patients experienced an average of 62.35 days with tumor marker levels below baseline.”
