[212Pb]VMT-α-NET is an SSTR2-targeted alpha-particle radiopharmaceutical being dose-found in a Phase 1/2a Bayesian escalation trial for unresectable/metastatic well-differentiated SSTR2+ NETs after ≥1 prior systemic therapy, excluding prior systemic PRRT (NCT05636618). Among 56 treated patients, there were no DLTs, grade 5 AEs, treatment-related discontinuations, serious renal events, or clinically meaningful treatment-related myelosuppression at median 40-week follow-up. In the efficacy subset (n=25), Cohort 2 had 9/23 RECIST responses (39%; 8 confirmed), above the ~13% ORR reported for ^177Lu-DOTATATE in NETTER-1, though the populations are not directly comparable.

Take home

Alpha-PRRT may be able to push response rates above beta-PRRT benchmarks, but this needs mature PFS and broader safety follow-up before the signal can be called more than early activity.
