In part 1 (dose level G) of the phase 3 MagnetisMM-6 trial (NCT05623020), 37 newly diagnosed, transplant-ineligible myeloma patients received subcutaneous elranatamab 76 mg Q4W plus daratumumab and lenalidomide. At the December 23, 2024 data-cut (median follow-up 4.6 month) the confirmed investigator-assessed ORR was 91.9% (36/37 responders, two pending), with 81% ≥VGPR. Grade ≥3 neutropenia was 68%, all grade 1-2 CR was 62%, and there was one fungal-pneumonia-related death. An April 1, 2025 update with 7.9 mo follow-up showed ORR 97.3%. Benchmark DRd in the MAIA study posts 92.9% ORR and 47.6% CR, suggesting the bispecific add-on matches early response rates while deepening VGPR/CR without excess toxicity.
Take home
Longer follow-up will show whether elranatamab can push front-line depth and durability beyond DRd, potentially giving frail patients CAR-like efficacy in an off-the-shelf monthly shot.
