Talquetamab is a GPRC5D×CD3 T-cell–redirecting bispecific antibody, combined here with daratumumab, an anti-CD38 monoclonal antibody, with or without pomalidomide, an immunomodulatory agent. In the Phase 3 MonumenTAL-3 trial (NCT05455320) in relapsed/refractory multiple myeloma after ≥1 prior line, Tal-DP (n=287) and Tal-D (n=287) both beat DPd (n=290): 24-month PFS was 81.3% and 77.6% vs 51.2%, respectively, with HR 0.28 and 0.33; ORR was 88.2%/88.5% vs 77.6%, ≥CR 71.1%/69.0% vs 34.5%, and MRD-negative CR 52.3%/46.3% vs 15.9%. Serious AEs were common across all arms (63.0%/52.6%/53.7%), fatal AEs were 1.8%/4.0%/4.6%, and the key comparison should be the strong internal DPd control here—not only historical APOLLO, where DPd produced median PFS ~12.4 months versus Pd.

Take home

The big story is not just that talquetamab combinations beat DPd—it is that Tal-D appears very close to Tal-DP, raising the possibility of a pomalidomide-sparing, high-depth regimen if infections, CRS, skin/nail/oral toxicities, and treatment logistics remain manageable.
