AVA6000 is a fibroblast activation protein-cleavable peptide-drug conjugate designed to release doxorubicin preferentially in the tumor microenvironment. In the first-in-human Phase Ia/Ib trial (NCT04969835), the salivary gland cancer cohort across Phase Ia/Ib (n=30) had 2 partial responses and 25 stable diseases, for a 90% disease control rate, with median PFS not reached at >5 months’ median follow-up. Phase Ib safety included fatigue 56%, alopecia 52%, nausea 44%, anemia 26%, neutropenia 22%, grade 3–4 neutropenia 11%, and no grade 3–4 cardiac events or clinically meaningful LVEF decline despite high cumulative doxorubicin-equivalent exposure.

Take home

If the cardiac safety continues to hold, AVA6000 could make an old drug newly useful in salivary gland cancer, especially in patients where anthracycline exposure would usually be capped.
